Caregiver Consistency Checks

What does this report represent?

This report represents enrollment and profile data entered into DAISEY for WIHV organizations that may be missing or inaccurate. Use the lists of profiles provided and / or underlying data to determine which records need to be corrected. USING FILTERS

Use the following filters to limit the data to review:

Start Date & End Date, Grantee, Organization, Enrollment Type, Home Visiting Model, County, and Home Visitor.

Regarding the date filters: The report will display only actively enrolled clients within the selected start and end date, as determined by the enrollment and discharge dates on their profile form.

Form: Caregiver Profile

Questions considered: Enrollment Date (1st – 5th), Discharge Date (1st – 5th), Date of Birth, Primary Caregiver ID

Who is counted: All caregivers with a specific data quality issue, as defined on the left side of the report.

Notes:

  • The list on the right side of the report will default to listing the DAISEY ID of all caregiver profiles you have access to in DAISEY. Clicking the numbers of any bar on the left will update the list on the right to display only those clients.
  • Profiles listed in any of these consistency check categories should be reviewed, and if necessary, updated to ensure the information you have recorded for these clients is accurate.

Program Guidance:

  • All caregivers enrolled in home visiting services must have an enrollment date in their DAISEY profiles. Review the list of “Missing” and make any needed corrections. Caregivers who were referred to your program but never enrolled should appear in the “Missing” lists. No action is needed for these individuals.
